All of your personal, financial, and health information is stored with HIPAA approved safeguards in place. At any time you can access, request a copy of your health data, and/or have it sent to anyone of your choosing. If you ever request that your information be shared with anyone else, Life Strength Fitness is not liable for who can access it and/or what they may do with it.
For everyone’s protection, all personal information shared and discussed verbally or electronically is completely confidential unless permission is granted by the other to share it on a case by case basis. However, in the event of suspected illegal activity, pursuant to valid court order or subpoena; imminent or likely risk of danger to self or to others any relevant information will be shared with proper legal authorities.

Members of Life Strength Fitness LLC also agree that its employees or agents to have the right to take photographs, videotape or digital recordings during fitness testing or coaching session to use in any and all media, now or hereafter known, and exclusively for the purpose of promotion. Member further consents that their first name and identity may be revealed therein or by descriptive text or captions. I waive any rights, claims or interest I have to control the use of my identity in whatever media is used. I also understand that Life Strength Fitness is not responsible for any expense or liability incurred as a result of member’s participation.
